Requirements for becoming a BPJS First Level Health Facility

A. For Primary Clinics or equivalent must have:

1. Operational License

2. Practice License (SIP) for doctors / dentists and Practice Permits or Work Permits (SIP / SIK) for other health workers

3. Permit for Pharmacist Practices (SIPA) for Pharmacists in terms of clinics providing pharmaceutical services

4. Taxpayer Identification Number (NPWP) of the body

5. Cooperation agreements with networks, if needed

6. A statement of willingness to comply with the provisions related to the National Health Insurance

B. For Practice Doctors or Dentists must have:

1. Practice License

2. Taxpayer Identification Number (NPWP)

3. Cooperation agreements with laboratories, pharmacies and other networks

4. A statement of willingness to comply with the provisions related to JKN

C. For Puskesmas or equivalent must have:

1. Operational License

2. Practice Permit (SIP) for doctors / dentists, Pharmacist Practice License (SIPA) for Pharmacists, and Practice Permits or Work Permits (SIP / SIK) for other health workers;

3. Cooperation agreements with networks, if needed

4. A statement of willingness to comply with the provisions related to JKN

D. For Primary D Class Hospitals or equivalent must have:

1. Operational License

2. Practice License (SIP) for health workers who practice

3. Taxpayer Identification Number (NPWP) of the body

4. Cooperation agreements with networks, if needed

5. A statement of willingness to comply with the provisions related to the National Health Insurance

 

Note:

· Requirements and Forms are submitted to the local BPJS Health Branch Office.

· Primary Clinic is preferred to have a network with Dentists, and if the Primary Clinic has fulfilled the credentialing requirements, and in accorandce with the needs of the First Level Health Facilities in the Branch Office area, the Primary Clinic can enter into a Cooperation Agreement with the local Branch Office.
